- Educate patients and their family members on disease processes, nutrition, and self-care techniques.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Participate in discharge planning to ensure a smooth transition to other care settings.
- Current DE LPN license or compact state license.
- At least one year of LPN experience, preferably in home healthcare and med-surgical settings.
