Job Summary:

We are seeking an experienced Chronic Care Coach to join our team at El Centro Family Health. As a Chronic Care Coach, you will work collaboratively with care teams to develop and implement personalized care plans for patients with chronic conditions.

Key Responsibilities:

  • \
  • Provide education and support to patients on self-management techniques\
  • Collaborate with care teams to identify patient needs and develop care plans\
  • Coordinate follow-up appointments and communicate with patients and care teams\
  • Maintain accurate records of patient interactions and outcomes\\/ul>\

Requirements:

  • \
  • Bachelor's degree in healthcare or related field (Associate's degree with experience considered)\
  • No prior experience required; however, 6 months of clinical experience preferred\
  • Tuberculin Skin Test (TST) and Hep B Vaccine or Declination required\\/ul>\

Preferred Qualifications:

  • \
  • Associate degree in health education or related field\
  • 6 months of clinical experience in a multi-cultural setting\\/ul>\
