We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Novant Health. As a Registered Nurse, you will play a vital role in providing high-quality patient care and coordinating home health services.
About the Role:- Conduct admissions, recerts, ROC, and discharges for assigned case load.
- Conduct routine nursing visits as needed.
- Assess patient needs and coordinate patient care between patient, caregiver, physician, and other disciplines.
- Responsible for completion of all home health documentation.

Requirements:- Graduate of an accredited school of nursing required.
- Associate's or Bachelor's degree in nursing.

- Minimum 1 year of home health or related experience required.
