About the Role:

The Senior Staff Nurse will play a key role in supporting the Home Team Clinical Director with coordination of team functions to effectively promote quality end-of-life care in the home setting. This position requires a strong nursing professional who can adapt to changing patient needs while ensuring patient care efficiency, effectiveness, and satisfaction.

Responsibilities:

  • Promote and practice the Hospice mission and core values, following all policies and procedures of Tidewell Hospice.
  • Assist the Clinical Director with scheduling home team staff, adapting to patients' needs while ensuring patient care efficiency, effectiveness, and satisfaction.
  • Respond to patient and family phone calls, evaluate the need, provide recommendations based on the established plan of care, and assist with assigning colleague visits when necessary.
  • Directly supervise all Hospice Aides on the team, including supervision, coaching, mentoring, corrective action as needed, including completion of annual performance appraisals.
  • Recognize, reinforce, and enhance the role of all disciplines in the Interdisciplinary Group by acting as a role model, mentor, and first-line nursing resource. Provide education to IDG members regarding nursing aspects of patient/family care.
  • Collaborate and communicate effectively with associate medical director, clinical teams, admission team, liaisons, and all colleagues residing at the site.
  • Assist the Clinical Director with facilitating the Interdisciplinary Group review process in collaboration with the team physician.
  • Ensure compliance with all CHAP, State, Federal, and local government regulations; maintain a strong position against fraud and abuse.
  • Assist the Clinical Director with ongoing review of patient documentation to ensure completeness, accuracy, and timeliness, and verify that the electronic medical record system (EMR) is being utilized appropriately.
  • Support the Clinical Director with monitoring patient ratios for team members, maintaining necessary staffing with census fluctuations.
  • In collaboration with the Clinical Director, participate in Quality Assessment Performance Improvement strategies and initiatives to assure quality of care and improved outcomes.
  • Assume responsibility for the home team in the absence of the Clinical Director, providing oversight of team functions related to staffing, patient/family needs, and any other required duties that emerge.

Requirements:

  • Must possess current and active Florida licensure as RN.
  • Minimum of 2 years recent community-based hospice or home health experience required.
  • An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.

Compensation:

The estimated salary for this position is $80,000 - $110,000 per year, depending on experience.
