Job Description

We are seeking a skilled Nurse Practitioner Home Visits professional to join our team in Denver, CO. As a Home-Based Medical Care Specialist, you will provide medical care and management to members served by the program, working closely with healthcare teams to develop and implement personalized care plans.

Responsibilities

  • Provide medical care and management to members served by the program
  • Develop and implement personalized care plans in collaboration with healthcare teams
  • Conduct initial comprehensive assessments and bi-annual follow-ups as needed
  • Prescribe appropriate diagnostics and interventions to avoid unnecessary transfers or acute admissions
  • Collaborate with UM staff and PCPs/facility staff to enable medical necessity determinations for requested medical services
  • Educate members and/or their legal representatives in disease processes affecting members and ways to manage them effectively
  • Support FCMs in developing and providing in-service educational programs to facility staff
  • Participate in management meetings, including case conference reviews, staff meetings, and quality improvement initiatives

Requirements

  • Completion of Nurse Practitioner accredited program
  • Current state RN license
  • Current NP license
  • Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
  • Computer skills, including Windows 2000 and MS Office Word Processing software
