Job Title: Patient Care Coordinator

We are seeking a highly skilled and compassionate Clinical Healthcare Professional to join our healthcare team.

About the Role

In this role, you will be responsible for creating and managing personalized care plans for individual patients. As a Clinical Healthcare Professional, you will operate with great attention to detail and take pride in observing and communicating effectively in your patient's best interest.

Key Responsibilities
  • Collaborate with colleagues to assess, plan, implement, or evaluate patient care plans
  • Administer medications and monitor patients for side effects or reactions
  • Document changes in patient conditions and report on changes
  • Maintain accurate records of patient care and discuss observations with the supervisor
Requirements
  • Currenlty licensed Registered Nurse (RN) in state of employment
  • Previous experience as a Registered Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver's license
  • First aid/CPR certification required
Compensation Information

$65,000 - $85,000 per year
