- Conduct thorough psychosocial evaluations to identify areas for improvement and drive effective discharge planning.
- Develop and implement comprehensive care plans in collaboration with the interdisciplinary treatment team.
- Maintain accurate records of patient interactions, progress, and outcomes.
- Communicate effectively with patients, families, and healthcare providers to ensure smooth care transitions.
- Bachelor's degree in Social Work from an accredited program required.
- Current license in Social Work in the State of South Dakota is mandatory.
