Role Summary:
The individual in this role is accountable for escorting clients to their medical appointments and ensuring that their medical records are accurately updated. This position plays a vital role in enhancing the independence of our clients by embodying the core values of Chrysalis: Respect, Safety, Fun, Mentoring, and Accountability.
Key Responsibilities:
- Provide transportation and accompany clients to their medical appointments.
- Exhibit punctuality and maintain consistent attendance to ensure proper supervision of clients.
- Remain alert and responsive, fostering effective communication with clients.
- Manage challenging behaviors of clients independently.
- Inform House Managers about clients' medical appointments.
- Ensure the upkeep of the company vehicle.
- Stay informed about clients' medical histories.
- Update clients' medical records in the database.
- Maintain confidentiality regarding clients' medical information.
- Adhere to the Provider Code of Conduct as per company and state regulations.
- Demonstrate flexibility in availability, as daily schedules may vary.
- Perform additional duties as assigned by the Medical Coordinator.
Qualifications:
- High school diploma or GED is required.
- Must be at least 21 years old.
- Pass a criminal background check and comply with state regulations annually.
- Ability to communicate effectively in English, both verbally and in writing.
- Must possess a valid driver's license and be eligible to operate a company vehicle.
- Maintain compliance with the Chrysalis Driving Requirement Policy.
Training and Certifications:
- Complete all initial and ongoing training requirements within designated timeframes.
- Obtain annual certifications in CPR, First Aid, and OSHA.
- Achieve and maintain SOAR certification.
Essential Skills and Abilities:
- Strong interpersonal and communication skills to effectively interact with clients and staff.
- Proficient in Microsoft Word.
- Exhibit professional phone etiquette and public relations skills when engaging with various stakeholders.
- Demonstrate excellent organizational capabilities.
- Ability to comprehend and document medical notes accurately.
- Work efficiently with minimal supervision.
Physical Requirements and Work Environment:
The physical demands and work environment characteristics described are representative of those encountered while performing the essential functions of this role. Reasonable accommodations may be made to enable individuals with disabilities to perform these functions.
Physical Demands: While executing the responsibilities of this position, the employee is required to sit, talk, and hear; utilize hands to handle objects; reach with arms, bend, stoop, and/or crouch to file or make copies. Occasionally, the employee may need to walk or stand and lift or move over 15 pounds.
Work Environment: The employee regularly drives a motor vehicle and is exposed to an indoor environment with moderate temperatures and minimal noise levels. The employee may encounter various environments, including those involving aggressive individuals and potential exposure to bodily fluids.
