Position Overview:

We are seeking a skilled Advanced Practice Provider to enhance our established Primary Care practice. This role involves conducting thorough patient evaluations and determining diagnoses based on comprehensive assessment data and the latest scientific research.

Key Responsibilities:
  • Develop and implement individualized care plans in alignment with established protocols and current clinical guidelines.
  • Deliver education and support to patients, families, and community groups.
  • Work within a structured schedule, Monday through Friday, from 8 AM to 5 PM, ensuring a favorable work-life balance with no on-call duties or weekend shifts.

Qualifications:

To be considered for this position, candidates must possess:

  • A Master's degree or higher in nursing or physician assistant studies from an accredited institution
  • A current Illinois license as a nurse practitioner or physician assistant
  • Certification from the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
  • A minimum of two years of experience as an Advanced Practice Provider in primary care or a related area is preferred
  • Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
  • Proficiency in electronic medical record (EMR) systems
