About the Role:
You will empower patients and families to reach healthcare goals by educating them on disease management. As a case manager and clinical leader, you will oversee an interdisciplinary team and provide high-quality patient care. To succeed in this role, you must have a RN license in the state where you work, two years of experience as a registered nurse, home health experience preferred, current driver's license, and current CPR and BCLS certification.
