The Bayview Hunters Point Foundation is seeking a skilled and compassionate Residential Case Manager I to join our team. As a key member of our staff, you will be responsible for providing comprehensive case management services to residents, helping them achieve stable housing and self-sufficiency.
Key Responsibilities- Conduct initial assessments and intake interviews with new residents to understand their needs and goals.
- Develop and implement individualized service plans to address residents' needs and promote self-sufficiency.
- Provide ongoing case management, including regular check-ins and progress evaluations, to ensure residents are meeting their goals.
- Maintain accurate and up-to-date case files and documentation to track resident progress.
- Connect residents with community resources and services, such as housing, employment, healthcare, and education, to support their well-being.
- Advocate on behalf of residents to ensure they receive necessary services and support.
- Coordinate with external agencies and service providers to facilitate access to resources.
- Offer crisis intervention and support to residents experiencing immediate needs or emergencies.
- Facilitate support groups, workshops, and life skills training for residents to promote personal growth and development.
- Assist residents with applications for benefits, housing, and other services to support their stability.
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field (preferred).
- Experience in case management, preferably in a homeless shelter or similar environment.
- Ability to work effectively in an open office environment surrounded by moderate noise and distractions.
- Excellent communication and interpersonal skills to work collaboratively with residents, staff, and external partners.
- Ability to maintain confidentiality and adhere to ethical standards in all aspects of case management.
