Job Summary:

The RN Nurse Navigator coordinates patient care across the continuum, ensuring seamless transitions between healthcare settings. This role requires strong communication and collaboration skills, with a focus on patient-centered care and quality outcomes.

Key Responsibilities:
  • Facilitate patient flow and coordinate appointments with multidisciplinary teams.
  • Identify community needs and provide education, screening, and support services.
  • Work with healthcare teams to ensure safe handoffs and coordination of care.
  • Trend data and outcomes to identify gaps in patient care.

Requirements:
  • Registered Nurse (RN) with 1-3 years of experience in the specialty.
  • CPR certification and Act 34, Act 33, and Act 73 clearances.
  • Preferred: Bachelor's Degree in Nursing and certification in area of expertise.
