  • Assist patients with daily living activities, such as hygiene, mobility, and elimination, as outlined in their care plan.
  • Maintain a safe environment for patients, using equipment properly and safely.
  • Practice Universal Infection Control Precautions in all patient interactions.
  • Perform or assist with home maintenance tasks, including light housekeeping and meal preparation, as prescribed in the care plan.
  • Monitor patients' fluid and nutrition intake, temperature, pulse, respiration, skin, and basic bodily functions, and record/report accordingly.
  • Use safe transfer, ambulation, normal range of motion, and positioning techniques.
  • Identify the need for intervention by other home health team members and report to the RN or Director of Clinical Services.
  • Respect patients' autonomy and confidentiality.
  • Document care provided and patient response, submitting documentation in a timely manner.
  • Work within the limits of your preparation and experience.
  • Ensure compliance with local, state, and federal laws, Medicare regulations, and agency policies and procedures.
  • Manage a caseload and deliver care in a cost-effective manner.
  • Meet or exceed 365 Health Services' established service standards.
  • Conduct business activities in a professional and ethical manner.
  • Use supplies and equipment efficiently.
  • Complete a minimum of 12 hours of in-service training per year.
  • Maintain regular attendance.
  • Adhere to 365 Health Services' Code of Conduct.
  • Interact with staff in a positive manner, supporting the company's mission and vision.
  • Conduct business activities in a professional and ethical manner.
  • Comply with 365 Health Services' general orientation and agency policies and procedures, including confidentiality and HIPAA guidelines.
