Key Responsibilities- Develop and implement individualized care plans for residents, taking into account their unique needs and preferences.
- Provide direct care to residents, including administering medications, performing wound care, and assisting with daily living activities.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Monitor and report changes in resident condition, identifying potential health risks and taking proactive steps to mitigate them.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Current RN licensure in the state of Ohio.
- Minimum 1 year of experience in long-term care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
- Ability to lift, transfer, and reposition residents as needed.
