Job Summary

We are seeking a highly skilled and experienced RN - Mechanical Heart Coordinator to join our team at Providence Sacred Heart Medical Center. The successful candidate will be responsible for coordinating and managing the transplant-related activities of patient caseload across the continuum of care.

Key Responsibilities
  • Coordinate patient evaluation, waiting list registration, and patient readiness for deceased donor transplants.
  • Arrange transplant admission, monitor clinical pathway progression, and provide post-discharge follow-up.
  • Provide patient and family education, professional and peer education, and participate in the development of patient education programs.
  • Maintain 24-hour on-call coverage for organ offers and other transplant needs.
  • Participate in outreach clinics with physicians, recipient selection for placement on the UNOS wait list, and communicate decisions to patients, nephrologists, and dialysis programs.
  • Provide inpatient and outpatient clinic transplant education, ongoing education, monitoring of labs, and arranging outpatient infusion and radiologic procedures.
  • Communicate and set up initial post-transplant visits with primary transplant physicians upon return to recipients' homes.
  • Assist with program development and awareness, research, and other duties as delegated.
Requirements
  • Bachelor's Degree in Nursing
  • Washington Registered Nurse License
  • Remains up to date on UNOS-OPTN and CMS regulations
  • 5 years of Acute care nursing experience
Preferred Qualifications
  • Certified Clinic Transplant Coordinator (CCTC) certification
  • 3 years Current experience in Cardiology nursing, including transplantation
