Job Title: Social Worker MSW

Providence Health & Services is seeking a skilled Social Worker MSW to join our team at Providence Milwaukie Hospital in Milwaukie, OR.

About the Role

We are looking for a compassionate and experienced Social Worker to provide comprehensive biopsychosocial/spiritual assessment, diagnosis, and treatment/interventions/advocacy of patients and their support systems. The ideal candidate will have a Master's Degree in Social Work and be licensed as a Clinical Social Work Associate in Oregon.

Responsibilities
  • Provide comprehensive biopsychosocial/spiritual assessment, diagnosis, and treatment/interventions/advocacy of patients and their support systems.
  • Collaborate with the patient and support system(s) to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT).
  • Perform duties in a manner that respects patient self-determination that reduces health disparities/barriers to care and adheres to the NASW code of ethics and Standards of Practice (SOP).
Requirements
  • Master's Degree in Social Work
  • Within 60 days of hire: Oregon Clinical Social Work Associate License
Preferred Qualifications
  • 2 years of Clinical work experience within healthcare, hospice, and/or bereavement.
