Patient Care Assistant II

As a Healthcare Support Technician, you will play a critical role in our interdisciplinary patient care services team, working closely with registered nurses and other healthcare professionals to ensure seamless delivery of care.
Job Summary
This role involves providing direct patient care, collecting vital signs, administering medication, and performing routine tests and procedures. You will also be responsible for maintaining accurate records, communicating effectively with patients and families, and participating in quality improvement initiatives.
Key Responsibilities
- Patient Assessment: Conduct thorough assessments of patients' physical and emotional needs, identifying areas for support and intervention.
- Care Coordination: Collaborate with multidisciplinary teams to develop and implement individualized care plans, ensuring continuity of care.
- Communication: Communicate effectively with patients, families, and healthcare professionals to ensure timely exchange of information and coordination of care.
Requirements
- High school diploma or equivalent required
- Current Maryland state certification as a Certified Nursing Assistant (CNA) or Geriatric Nursing Assistant (GNA) required
- One (1) year of previous experience preferred
- Basic Life Support (BLS) certification required
Salary and Benefits
- Average salary range: $18.03-$26.51 per hour
