About the Role

We are seeking a highly skilled and compassionate Case Manager Level III to join our team at Urban Strategies. As a key member of our Post Release Services team, you will be responsible for providing intensive case management services to children and their sponsors, ensuring their safety and well-being in a crisis.

Key Responsibilities
  • Manage an assigned caseload of post release services, including Levels I, II, and III.
  • Comply with US and ORR policies, procedures, and mandated timeframes.
  • Schedule and conduct interviews with care provider staff, children, and sponsors.
  • Assess safety on an ongoing basis and identify, verify, and make recommendations for additional support, resources, and information to meet a child's needs.
  • Maintain telephone contact and conduct virtual and in-person home visits with children and sponsors.
  • Provide crisis intervention services directly, safety planning as needed, and connect families to services to ensure child safety and well-being in a crisis.
  • Collaborate with children and sponsors, as well as available informal support and access to resources, to develop a plan to ensure the safety of the child and family.
  • Act as a point of contact or liaison for the child, sponsor, and external agencies.
  • Utilize trauma-focused approaches and therapeutic support to children and families with significant needs.
  • Provide culturally and linguistically appropriate services and comprehensive clinical case management services through screenings, assessments, evaluations, and providing psychoeducation to children and their sponsor.
  • Maintain appropriate documentation, safeguard confidential records, and protect personally identifiable information according to program regulations.
  • Identify and report concerns related to children and sponsors, life-changing events, and significant incidents to the supervisor and ORR.
  • Act as a mandated reporter and report abuse/neglect to child protective services.
  • Participate in training, meetings, and maintain quality improvement initiatives as recommended.

Requirements
  • A Master of Social Work (MSW) or an equivalent degree in psychology, sociology, or other behavioral science or social service field.
  • Relevant behavioral science in which direct clinical experience is a program requirement.
  • A bachelor's in one of the aforesaid fields with at least 5 years of clinical experience and may be licensed eligible.
  • Must be skilled in writing professional, thorough assessment reports and experience working with immigrant populations and has provided trauma-informed clinical interventions for youth.
  • Knowledge of family preservation, kinship care, trauma-informed practices, child welfare policies and procedures, and relevant state and federal laws and regulations.
  • Experience in providing Post Release Services and working with immigrant populations.
  • Effective communication and interpersonal skills to successfully interact with families, caseworkers, and other stakeholders.
  • Ability to handle sensitive and confidential information with discretion.
  • Fluent in English and Spanish (verbal/written). Proficiency in Microsoft Office and data entry systems.
  • A valid driver's license, available to travel to conduct in-person visits and to different areas or regions nationally.
