Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team. As an LPN, you will be responsible for providing skilled nursing and rehabilitation care to our guests. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each guest.

Responsibilities
  • Provide skilled nursing and rehabilitation care to guests, including administering medications, performing wound care, and providing physical therapy
  • Assess the health status of guests and notify the physician of changes in status
  • Implement new orders and develop a plan of care in collaboration with the interdisciplinary team
  • Supervise the care provided by Certified Nursing Assistants (CNA)/State-Tested Nursing Assistants (STNA) and other team members
  • Provide education and support to guests and their families
Qualifications
  • Current Licensed Practical Nurse (LPN) licensure in the state
  • 1-3 years of experience in a long-term care setting preferred
  • CPR certification
