Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As an LPN at Cottonwood Healthcare and Rehabilitation Center, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure their physical, emotional, and social well-being.

Responsibilities
  • Provide direct care to residents, including administering medications, monitoring vital signs, and performing routine checks.
  • Develop and implement individualized care plans, working closely with residents, families, and healthcare professionals to ensure their unique needs are met.
  • Collaborate with our interdisciplinary team to coordinate care, including RNs, therapists, and other healthcare professionals.
  • Communicate effectively with residents, families, and staff to ensure open and transparent communication.
Requirements
  • Current LPN license in good standing.
  • Compassionate and empathetic approach to care.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
