About the Role:
As a Patient Care Specialist, your primary responsibility will be to deliver compassionate care and support to individuals by promoting participation in group activities, social interactions, and various therapeutic initiatives. You will work collaboratively with a team of healthcare professionals to assess and implement optimal care strategies. Your role will also involve ensuring a safe and secure environment through effective de-escalation methods while safeguarding the well-being of patients.
Key Responsibilities:
- Maintain the safety and security of residents, which may include managing challenging behaviors when necessary.
- Accompany residents to different activity and therapy locations within the facility and off-site for medical, dental, or legal appointments, ensuring their safety at all times.
- Assist in executing individualized treatment plans for residents.
- Monitor residents during meals, classes, and other communal areas to ensure their well-being.
- Observe, document, and report on the condition and behavior of residents accurately.
- Utilize therapeutic interventions grounded in de-escalation and redirection techniques to manage behaviors effectively.
- Support residents in maintaining their living spaces, personal belongings, and hygiene.
- Engage residents in social and recreational activities, fostering a spirit of cooperation and community.
- Ensure security protocols are followed, including visitor checks and room searches for safety compliance.
This position may require mandatory overtime.
Qualifications:
- High School Diploma or High School Equivalency (HSE).
- Certification or willingness to obtain certification in CPR/AED for professional rescuers.
- Knowledge of psychiatric nursing care practices, including medication administration.
- Ability to maintain accurate records and reports.
- Capability to respond effectively in emergencies and manage challenging behaviors.
- Understanding of behavior modification techniques and safe restraint methods.
- Familiarity with institutional procedures and resident behavior management.
- Ability to assist residents with daily living activities and housekeeping tasks.
- Proficient in following both oral and written instructions.
Supervisory Responsibilities:
This position does not entail direct supervision of staff.
