About the Role
We are seeking a skilled Admissions Nurse to join our team at Bethesda Health. As a key member of our admissions team, you will be responsible for ensuring a seamless transition for patients and their families as they enter our care. Your expertise will be instrumental in assessing, planning, implementing, and evaluating patient plans of care, while providing exceptional customer service and communication with families, nursing home staff, social workers, and other team members.
Key Responsibilities
• Conduct thorough head-to-toe assessments to identify patient needs and develop individualized plans of care
• Coordinate communication with families, nursing home staff, and other team members to ensure a smooth admission process
• Complete initial plans of care, medication lists, and other necessary documentation in a timely and accurate manner
• Verify physician orders and ensure that all necessary treatments and orders are in place
• Offer and administer immunizations as needed, while maintaining accurate records
• Demonstrate expertise in nursing practice, administrative performance, and communication skills
Requirements
• Graduate of an accredited school of nursing
• Current nursing license in good standing in the state of employment
• Minimum two years of varied work experience as a professional nurse, preferably in a post-acute setting
• Strong administrative and communication skills, with the ability to work effectively in a fast-paced environment
