Job Summary

We are seeking a highly motivated Pharmacy Technician to join our team as a Transition of Care Specialist. In this role, you will work closely with patients, pharmacists, and healthcare providers to ensure a safe and effective medication transition.

Key Responsibilities
  • Collect and verify patient medication history, including allergies and medications
  • Communicate with patients, families, nurses, physicians, and other healthcare professionals to obtain accurate medication lists
  • Work closely with the emergency department's admitting clinical teams to ensure a smooth transition of care
  • Operate electronic health records to obtain patient medication history information
  • Contact outside medical facilities to obtain patient medication history information
Requirements
  • High school diploma or equivalent required
  • Massachusetts pharmacy technician registration and national pharmacy certification by Pharmacy Technician Certification Board (PTCB) required
  • Three years pharmacy experience required, with both community and inpatient hospital experience preferred
  • Must be proficient in math and able to use a computer for data entry
