Job Summary:

The Physical Therapist will be responsible for assessing patients' physical abilities, developing and implementing individualized treatment plans, and providing ongoing care and support to ensure optimal patient outcomes.

Responsibilities:
  • Conduct comprehensive assessments of patients' physical abilities and develop personalized treatment plans
  • Provide one-on-one care and support to patients in their homes, focusing on rehabilitation and recovery
  • Collaborate with healthcare teams to ensure seamless care coordination and communication
  • Participate in IDG meetings and care conferences to discuss patient progress and adjust treatment plans as needed
  • Document patient progress and maintain accurate records
Requirements:
  • Current licensure as a physical therapist in the state of Michigan
  • At least one year of experience as a physical therapist in the past five years
  • Active CPR certification
  • Reliable transportation with a valid state driver's license and car insurance
