* Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
* Maintain accurate and detailed records of patient care and progress
Requirements:
* 1+ year of current skilled home health experience
* LPN license (required)
* AHA BLS certification (required)
* High school or equivalent (preferred)
* LPN acute care experience (2 years required)
