Job Summary:
Purdue University, a leading institution in education and research, is seeking a skilled Referral Coordinator to join its team. The successful candidate will work under the supervision of the Director of Quality, ensuring seamless coordination of care for patients with complex healthcare needs.
Key Responsibilities:
- Manage and coordinate referrals for patients requiring specialty care and testing.
- Communicate effectively with patients, providers, and partners to ensure timely and efficient coordination of care.
- Utilize our referral tracking application to document and track referral activity, ensuring accurate and up-to-date information.
- Identify and communicate individual partner needs and processes to the team, promoting effective collaboration and coordination.
- Ensure the closing of the referral loop by receiving follow-up actions and results/reports, maintaining a high level of patient satisfaction.
- Maintain an up-to-date list of referral partners and processes, ensuring seamless communication and coordination.
- Provide additional assistance to the Referral Coordination Team as needed, supporting the delivery of high-quality patient care.
- Participate in regular staff meetings and training, staying up-to-date on best practices and industry developments.
Requirements:
- High school diploma or equivalent.
- Four years of experience in a medical setting, with a combination of related education and experience accepted.
- Excellent oral and written communication skills, with the ability to work effectively as a member of a multidisciplinary team.
- Analytical and problem-solving skills, with the ability to organize and prioritize work.
- Confidentiality and discretion required, with a strong commitment to the mission of Purdue University and positive customer service skills.
Preferred Qualifications:
- Experience in case management or community services.
- Understanding of the healthcare delivery system, with knowledge of Medicaid, Medicare, and other third-party payers.
- Computer knowledge and experience working in an electronic health record.
Core Functions:
- Management and coordination of referral orders for specialty care and testing.
- Communication of referral activity and progress to patients, providers, and partners.
- Identification and communication of individual partner needs and processes.
- Ensuring the closing of the referral loop.
- Maintenance of referral partner and process lists.
Other Duties:
- Compliance with federal and state laws, Purdue University policies, procedures, rules, and regulations.
- Representation of Purdue University in a professional and courteous manner.
- Compliance with HIPAA rules.
- Attendance at meetings and training sessions as required.
- Representation of the organization at conferences and seminars as needed.
