Home Care Licensed Practical Nurse Job

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Sheboygan, providing high-quality care to patients in their homes. Job Summary

The Home Care Licensed Practical Nurse is responsible for providing direct nursing care to patients in their homes, as prescribed by the physician. This includes recording clinical and progress notes, coordinating patient care activities, and working collaboratively with the healthcare team to provide the highest quality of patient care.

Responsibilities
  • Provide high-quality clinical services within the scope of practice, as defined by state laws governing nursing practice.
  • Document patient visits thoroughly and completely, following regulatory and payer requirements.
  • Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
  • Ensure confidentiality of patient health information, as per agency policy.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
