Job Title: Occupational Therapist

We are seeking a skilled Occupational Therapist to join our team at Sunnyvale Gardens Post Acute. As an Occupational Therapist, you will play a vital role in helping our patients achieve their maximum potential and independence.

Job Summary:

The Occupational Therapist will be responsible for developing and implementing effective treatment plans for patients, working closely with the nursing department and other healthcare professionals to ensure seamless care. This is a full-time, part-time, and PRN opportunity with a competitive salary and benefits package.

Key Responsibilities:
  • Develop and implement individualized treatment plans for patients, including goal setting and progress monitoring.
  • Conduct thorough assessments of patients' physical and cognitive abilities, identifying areas for improvement and developing strategies for rehabilitation.
  • Collaborate with the nursing department and other healthcare professionals to ensure comprehensive care and effective communication.
  • Supervise Occupational Therapy Assistants in direct patient care and patient-related activities, following state practice act.
  • Communicate with supervisors and other healthcare team members regarding patient progress, problems, and plans.
  • Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
  • Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, and other required documentation.
  • Instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services.
  • Participate in discharge planning and recommend and facilitate the ordering of necessary durable medical equipment for patients.
  • Report any problems with department equipment to ensure it is maintained in good working order.
