Overview

The role involves direct engagement with clients of the Assertive Community Treatment (ACT) Team, utilizing the evidence-based ACT model developed by NAMI. The primary focus is to deliver comprehensive and holistic services aimed at achieving client objectives related to mental health, physical health, housing, employment, social interactions, and other personal goals identified by the client. Responsibilities include conducting assessments and formulating treatment plan objectives. Collaboration with clients and team members is essential to meet the outcome targets established by Polk County Health Services (PCHS). Additionally, the position entails providing medication education and organization for ACT clients and case managers, processing orders, completing prior authorizations, and liaising with medication prescribers to ensure clients receive necessary psychiatric medications.

Responsibilities

Client Engagement: Assess client needs and establish goals that promote independence within the community. Provide skills training and collaborate with clients and team members to explore alternatives to hospitalization, higher levels of care, homelessness, and incarceration. Advocate for clients to address their needs and interests. Employ critical thinking to make independent decisions while maintaining accountability for the services provided. Understand the impact of mental illness and relate respectfully to clients with diverse abilities and diagnoses. Meet clients in their homes or public settings, with the ability to drive safely and transport clients as needed. Demonstrate teamwork skills, including problem-solving and cooperation. Be available for on-call shifts during evenings and weekends, which includes managing the 24-hour on-call client crisis line and delivering medications, as well as providing crisis triage services via phone and potentially on-site.

Medical Responsibilities: Under the guidance of the team medication prescriber and in collaboration with other registered nurses, develop, revise, maintain, and oversee team policies and procedures related to psychopharmacologic and medical treatment, including transcribing, administering, evaluating, and documenting prescribed psychotropic medications. Evaluate and document the effectiveness, complications, and side effects of psychotropic medications. Coordinate necessary lab work according to established protocols. Organize and manage the process of delivering medications to clients and integrate medication into individual treatment plans. Manage pharmaceuticals and medical supplies. Schedule and conduct nursing assessments of client physical health, making appropriate referrals to community physicians for further evaluation and treatment, and coordinate psychiatric treatment with medical care. Administer IM injections as per protocol and ensure proper disposal of medications according to guidelines. Follow up on orders, lab results, and medication prescriptions for all clients, ensuring attendance and follow-up with psychiatric prescribers.

Documentation: Complete all required documentation within established timelines. Prioritize client needs and fulfill necessary written documentation. Demonstrate an understanding of all funder and accreditation standards. Track and document monthly outcome objectives.

Qualifications

Education: Must be a registered nurse or licensed practical nurse, with a preference for candidates with mental health experience.

Experience: Minimum of one year working with the served population.

Licenses/Certifications: Valid driver's license required for work-related driving. Must be licensed in the state of Iowa as an RN or LPN in good standing.

Knowledge/Skills/Abilities: Proficient in speaking, reading, and writing fluent English. Ability to comprehend common terminology in the mental health field. Competent in entering documentation in an EHR and using Microsoft Office. Capable of effectively presenting information and responding to inquiries from clients, medical professionals, management, staff, and the public.

Other: Familiarity with standard equipment used to perform essential job functions.
