• Complete admission paperwork and ensure accurate documentation of resident care services
• Provide advice and support to residents and their families, promoting a positive and empowering experience
• Monitor residents and administer medication and treatments as needed, ensuring the highest level of care and safety
• Document resident care services by charting in resident and department records, maintaining confidentiality and adhering to infection-control policies and protocols
• Resolve resident problems and needs by utilizing multidisciplinary team strategies, promoting a collaborative and patient-centered approach
• Maintain resident confidence by keeping information confidential and adhering to facility philosophies and standards of care
• Track and report infection control measures, ensuring a safe and healthy environment for residents and staff
**Requirements:**
• Current NYS State NP/PA License
• Long-Term Care Experience Preferred
• Must be wound and ostomy certified
• Proven experience with skin graft procedures
• Solid computer skills; working knowledge of MS Office
• Excellent communication skills
• Friendly and a team worker
