Responsibilities:
- Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop individualized plans of care.
- Coordinate total plan of care with other healthcare professionals, ensuring continuity of patient care.
- Provide skilled nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
- Document accurate, complete, and timely patient information in collaboration with physician orders.
- Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
Requirements:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
