We are seeking a compassionate and experienced Hospice Nurse Specialist to join our team at Phoenix Home Care and Hospice. As a Hospice Nurse Specialist, you will be responsible for coordinating and overseeing the provision of hospice care to patients and their families. Your extensive nursing experience, particularly in community health education and practice, will enable you to direct the immediate services required to address the unique needs of individuals and families in their homes and communities.
Responsibilities:- Meeting the healthcare needs of hospice patients by completing as needed and routine visits
- Working closely with the Hospice team, physicians, and community partners to deliver optimal care
- Triaging emergent needs and collaborating with physicians to provide comfort and quality care to each patient
- Maintaining accurate, up-to-date records
- Providing services in accordance with agency policies and regulations that define scope of practice
- Participating in in-service programs
- Demonstrating strong communication and customer service skills
- Hospice experience preferred
- Missouri Nursing License or Compact
- Strong organizational and self-management skills
- Strong and compassionate customer service skills
- Valid driver's license
