We are seeking a highly skilled and compassionate Advanced Practice Clinician to join our Senior Community Care team at Optum. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to our most vulnerable populations.
Key Responsibilities- Conduct comprehensive assessments of patients' medical, social, and functional needs
- Develop and implement individualized care plans to address patients' complex health needs
- Manage patients through acute medical situations, exacerbations of chronic conditions, and short-term sub-acute rehabilitation episodes
- Ensure accurate and complete documentation of patient information, including ICD-10 conditions
- Collaborate with the interdisciplinary care team to coordinate patient care and services
- Provide advanced illness and end-of-life discussions with patients and their families
- Offer counseling and education to patients and caregivers on disease management, self-care, and health promotion
- Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program (NP) or accredited Physician Assistant degree program (PA)
- Active and unrestricted license in the state of practice or ability to obtain by start date
- Active certification as a Nurse Practitioner or Physician Assistant through a national board
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
- Access to reliable transportation
- 1+ years of clinical experience in a long-term care setting
- Experience in meeting the medical needs of patients with complex behavioral, social, and functional needs
- Understanding of geriatrics and chronic illness
- Understanding of advanced illness and end-of-life discussions
- Proficient computer skills, including documentation of medical information in electronic medical records
