We are seeking a skilled Licensed Vocational Nurse to provide high-quality care to patients in hospice settings. The ideal candidate will possess a valid LVN license, excellent communication skills, and the ability to work independently and as part of a team.

Key Responsibilities:
  • Conduct thorough patient assessments to identify care needs.
  • Provide clear instructions to patients on prescribed medications.
  • Monitor and record vital signs, reporting changes to the medical team.
  • Offer emotional support to patients and their families.
  • Perform simple wound care as needed.
  • Maintain accurate patient records in accordance with established standards.
  • Provide PRN visits and medication reconciliation as required.
  • Ensure overall patient well-being and provide necessary care.
