Responsibilities:
  • Assess and evaluate patient therapeutic, rehabilitative, and functional status to develop a comprehensive plan of care.
  • Provide physical therapy services to patients according to a written physician's plan of care, including assessing muscle strength, mobility, gait, and range of motion.
  • Direct physical therapy treatment and instruct patients/caregivers in the use and care of therapeutic appliances.
  • Identify patient and family/caregiver needs for other home health services/hospice and refer as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals.
  • Participate in discharge planning for patients and provide physical therapy consultation to home families/caregivers when indicated.
Requirements:
  • BLS/BCLS certifications
  • Valid state license (where applicable)
  • SNF experience - Required
  • Candidate must be vaccinated
