Hospice Certified Nurse Assistant Position Summary

Capital Hospice & Palliative Care is seeking a dedicated and compassionate Hospice Certified Nurse Assistant to join our team. In this role, you will have the opportunity to make a meaningful difference in the lives of patients and families facing end-of-life care.

Main Responsibilities:
  • Provide personalized care and support to patients in their homes, focusing on promoting comfort, dignity, and well-being.
  • Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Deliver essential care services, including bathing, dressing, and feeding, as assigned by the patient's plan of care.
  • Monitor and report changes in patient condition to the RN/Case Manager and other relevant staff members.
  • Ensure accurate and timely documentation of all care provided and any notable events or concerns.
Essential Qualities:
  • State-approved training program completion.
  • High school diploma or equivalent required.
  • Excellent communication and interpersonal skills.
  • Ability to work independently with minimal supervision and maintain confidentiality.
  • Ongoing education in hospice care and related fields.
