The Role:

We are seeking a highly skilled and compassionate Patient Care Manager to join our team. In this role, you will play a pivotal part in ensuring the delivery of exceptional patient care by overseeing the management of our hospice program. This includes:

  • Maintaining a high-performing hospice program through effective management and oversight of patient care activities
  • Directing and teaching staff to ensure adherence to company policies and procedures, as well as compliance with local, state, and federal regulations
  • Responding to phone referrals and inquiries in a timely and professional manner
  • Serving on the Interdisciplinary Group (IDG) and facilitating IDG meetings

About You:

We are looking for a seasoned healthcare professional with at least three years of direct patient care experience and two years of experience in a hospice or home-based care setting. A graduate degree in nursing is preferred. The ideal candidate will possess:

  • Current Registered Nurse licensure in the state of residence
  • Demonstrated knowledge and compliance with accepted hospice principles and practice
  • Ability to apply knowledge of the unique needs of hospice patients and families from various socioeconomic backgrounds
