Job Title: Caregiver

Sweet Home Services LLC is seeking a compassionate and dedicated Caregiver to join our team. As a Caregiver, you will play a vital role in providing personalized support services to individuals with developmental disabilities.

Key Responsibilities:
  • Assist individuals with daily living activities, including personal hygiene, meal preparation, and mobility assistance.
  • Implement Individualized Service Plans (ISPs) and ensure all activities align with the consumer's personal goals and needs.
  • Provide companionship, emotional support, and promote social engagement and inclusion within the community.
  • Administer medication and monitor the health and wellness of the individuals.
  • Assist in transportation to appointments, recreational activities, and community outings.
  • Maintain accurate documentation of care, including daily logs, health records, and incident reports as required by DDD and company policies.
  • Follow prescribed behavioral support plans and interventions as outlined in ISPs.
  • Uphold the dignity and rights of individuals, ensuring that their choices and preferences are respected.
  • Participate in training sessions and maintain required certifications (e.g., CPR, First Aid).
Qualifications:
  • High School Diploma or GED required.
  • Must be 18 years of age or older.
  • Experience working with individuals with developmental disabilities preferred.
  • Valid Driver's License and access to reliable transportation.
  • Ability to pass a background check and other required screenings (e.g., Child Abuse Record Information (CARI)).
  • Must complete mandatory CPR, First Aid, and other required training through approved agencies, including training offered via the College of Direct Support.
  • Strong communication and interpersonal skills.
  • Compassionate, patient, and committed to supporting individuals with varying abilities.
