Job Summary
We are seeking a skilled Healthcare Physician to join our team at Oak Street Health, where you will play a vital role in delivering high-quality, patient-centered care to older adults in our community. As a Primary Care Physician with expertise in Geriatric Medicine, you will be responsible for assessing and diagnosing patients, prescribing treatments, and collaborating with other healthcare professionals to ensure seamless care delivery.

About the Role
This full-time position offers a competitive salary of $250,000 per year, plus an annual bonus based on quality metrics. Our innovative value-based care model rewards physicians for delivering exceptional outcomes and preventative medicine, rather than just volume of services. You will have the opportunity to work with a dedicated medical scribe and administrative support team to focus on what matters most – delivering top-notch patient care.

Responsibilities
As a Healthcare Physician at Oak Street Health, your key responsibilities will include:
  • Assessing and diagnosing patients in our local clinics
  • Overseeing and directing primary care services
  • Prescribing and administering pharmaceutical treatments and medication
  • Maintaining accurate Patient Electronic Medical Record data via Canopy and Greenway
  • Collaborating with regional and central leadership to meet health quality goals
  • Working closely with Practice Managers to direct and manage the center care team

Requirements
To be successful in this role, you must possess:
  • A Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree
  • Board certification in Internal Medicine or Family Medicine (or board eligible)
  • An active, non-probationary, unrestricted State License
  • An active DEA license
  • USA work authorization

Preferred Qualifications
While not required, we welcome applicants with experience practicing medicine among Geriatric populations, operating in Value-Based Healthcare Models, working in collaborative settings, or possessing fellowship training in Geriatrics.
