Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Copper Creek Cottages Memory Care. As an LPN, you will play a vital role in providing high-quality care and support to our residents, ensuring their physical, emotional, and social needs are met.

Responsibilities
  • Administer medications to residents in accordance with their individual care plans
  • Monitor and record residents' medical and medication charts/records
  • Assist residents with Activities of Daily Living (ADLs) as needed
  • Maintain positive relationships with residents, families, and physicians
  • Train and lead care staff
  • Attend and participate in scheduled in-services, training programs, and staff meetings
  • View incident reports related to health conditions and update resident care plans as needed
  • Perform Dr. and hospital follow-up and facilitate resident return process when necessary
  • Assist in additional nursing services as requested by management
Qualifications
  • Valid state-issued LPN license
  • Abide by all facility and state rules and regulations
  • Experience in direct patient care and assessments/observations of clinical conditions
  • CPR certified and knowledge of basic first aid
  • Must complete all state-required training to maintain licensure or certification
  • Must pass background check and drug screen
