Mental Health Case Manager

The Case Manager - Intensive plays a vital role in the South County Mental Health Center's mission to provide exceptional care to adults with severe and persistent mental illness.
Key Responsibilities:- Client Care: Conduct thorough assessment interviews with clients to understand their needs and develop personalized care plans.
- Accompany clients to and link them with community resources, such as housing agencies, social service organizations, and healthcare providers.
- Counsel clients and their families on relevant clinical issues, providing education and support to promote mental health and well-being.
- Perform regular home visits to monitor clients' progress and provide ongoing support.
- Advocate for clients by assisting with applications for services and communicating with providers to ensure timely access to care.
- Coordinate care with psychiatrists, therapists, medication clinics, and external services to ensure comprehensive support.
- Monitor treatment progress to ensure that needed services are provided in a timely manner.
- Attend staff meetings to stay informed about center policies and procedures.
- Document client progress, including progress notes, reviews, and reports, to maintain accurate records.
- Develop comprehensive service plans that address clients' unique needs and goals.
- Compose and type letters, correspondence, and other reports as needed.
- Complete staff activity reports, PB2, and functional assessment rating scales to track progress and identify areas for improvement.
- Achieve daily entries into the electronic medical record (EMR) system to ensure accurate and up-to-date client information.
Requirements:
- BA/BS in Social Sciences (Social work, psychology, sociology, etc.) from an accredited college or university.
- 1 year of experience working in mental health or a related field.
- Valid FL driver's license.
- PREFERRED EDUCATION & EXPERIENCE:
- MA/MS in Social Sciences (Social work, psychology, sociology, etc.) from an accredited college or university.
- 2 years of case management experience.
- Case Manager Certification (CBHCM) - Florida.
- Knowledge & Skills:
- Familiarity with 65E regulations, Baker Act, and Marchman Act.
- Ability to interpret and abide by confidentiality laws, abuse reporting guidelines, and client rights requirements.
- Proven track record of abiding by a code of ethical and practice standards.
- Basic counseling skills.
- Case management skills.
- Advanced communication skills, both written and verbal.
- Excellent organizational and time management skills.
- General typing and computer literacy skills.
- Proficiency in educating adults on illness, medication compliance, community resources, money management, and basic life skills.
- Experience with assessment and treatment planning.
- Capability to handle verbally or physically aggressive clients.
- Knowledge of psychiatric medication and community resources.
