Job Title: LPN Medication Care Manager

Job Summary:

We are seeking a highly skilled and compassionate LPN Medication Care Manager to join our team at Sunrise Senior Living. As a key member of our healthcare team, you will be responsible for providing high-quality care and services to our residents, ensuring their safety and well-being.

Responsibilities:
  • Medication Administration:
  • Administer medications and treatments in a safe and organized manner, adhering to the Six Rights of Medication Pass.
  • Review and document medication updates, ensuring accurate and timely administration.
  • Conduct regular audits to ensure medication carts are stocked and organized.
  • Collaborate with the Resident Care Director (RCD) and Wellness Nurse to ensure medication management is aligned with resident care plans.
Resident Care:
  • Notifying RCD of Resident Concerns:
  • Notify the RCD of any resident concerns or issues, ensuring prompt attention and resolution.
  • Attending Cross Over Meetings:
  • Attend daily Cross Over meetings to discuss resident care plans, medication management, and other relevant topics.
  • Conducting Service and Health Updates:
  • Conduct regular Service and Health Updates to ensure residents receive the highest quality care.
Risk Management and General Safety:
  • Partnership with Community Team:
  • Partner with the community team to ensure compliance with OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements.
  • Practicing Safety Procedures:
  • Practice safety procedures at all times, including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
Training and Contributing to Team Success:
  • Actively Participating in Team Meetings:
  • Actively participate in team meetings, including Town Hall, Department Team, Cross Over, Medication Technician, and others as directed by the Supervisor/Department Coordinator.
  • Attending Regular Training:
  • Attend regular training sessions, including those provided by the RCD and neighborhood coordinators.

Qualifications:

  • LPN, LVN, or state/province specific licensed nurse credential.
  • Maintain certifications as required by Sunrise and state/provincial regulations.
  • Previous experience working with seniors preferred.
  • Ability to make choices and decisions in the resident's best interest.
  • Ability to react and remain calm in difficult situations.
  • Ability to handle multiple priorities.
