The Rehabilitation Nurse Case Manager is responsible for delivering high-quality, patient-centered care by overseeing rehabilitative services to achieve optimal patient outcomes. This role involves:
- Providing orientation to patients, families, and caregivers regarding the case manager's role, rehabilitation philosophy, and criteria for continued care and discharge.
- Coordinating patient and family education, encouraging active participation in discussions about care plans, goals, and progress.
- Implementing treatment plans that align with the patient's strengths, needs, and preferences, ensuring their involvement throughout the rehabilitation journey.
- Documenting findings from discharge planning evaluations and psychosocial assessments, and communicating any social, financial, or discharge needs.
- Promoting consistent, positive interactions with patients to enhance their experience and satisfaction.
To be considered for this role, candidates must possess:
- Basic Cardiac Life Support certification obtained within 30 days of employment.
- A valid Registered Nurse license.
- A Bachelor's Degree in a relevant field.
- A minimum of 1 year of experience in a similar role.
