We are seeking a highly skilled and compassionate Home Health Visiting Nurse to join our team at HarmonyCares. As a key member of our healthcare team, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and improving health outcomes.
Responsibilities- Assess and evaluate patient needs, identifying mutually agreed-upon goals and developing individualized care plans to meet those needs.
- Provide intermittent skilled nursing services, including assessment, evaluation, procedures, teaching, and training activities as outlined in the patient Plan of Care.
- Communicate patient status and need for other disciplines to agency Clinical Supervisor and referring physician, ensuring seamless coordination of care.
- Supervise Home Health Aides and document patient care, maintaining accurate and comprehensive records.
- Participate in discharge planning, ensuring a smooth transition of care and promoting patient safety.
- Collaborate with other healthcare professionals to ensure comprehensive care and address patient needs.
- Current unencumbered State professional Nurse License.
- Valid driver's license and good driving record.
- Ability to make sound professional clinical judgment and assess patient needs.
- Excellent verbal and written communication skills, with proficiency in personal computer use, including email, clinical, word processing, spreadsheet, and presentation software.
- Active CPR Certification.
- One year of experience as a home care professional nurse, with competence in performing home care comprehensive assessments.
