Habilitation Specialist

We are seeking a Habilitation Specialist to join our team at Network Human Services. As a Habilitation Specialist, you will work closely with residents to develop and implement individualized care plans that promote independence, socialization, and overall well-being.

Main Responsibilities:

• Conduct assessments to identify resident strengths, needs, and goals.
• Develop and implement individualized care plans that promote habilitation and rehabilitation.
• Collaborate with interdisciplinary teams to ensure seamless delivery of care and services.
• Maintain accurate records of resident progress and care provided.
• Participate in ongoing education and training to enhance knowledge and skills.

Requirements:

• Diploma or degree in a related field (e.g., social work, occupational therapy).
• Experience working with individuals with disabilities or elderly population.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team environment.

Compensation:$42,000 - $50,000 per year
