Job Overview

We are seeking a highly skilled Care and Support Specialist to join our team. This role is a key component of our clinical programs' success, working directly with individuals with disabilities to help them gain essential skills for living, working, and recreating based on their personal preferences.

Responsibilities
  1. Develop and implement personalized recovery plans, promoting rehabilitation, wellness, and empowerment.
  2. Conduct comprehensive assessments, documenting findings and progress towards established goals.
  3. Implement strategies outlined in individual treatment plans, ensuring seamless integration of services.
  4. Collaborate with multidisciplinary teams to develop and monitor treatment planning goals across all life domains.
  5. Maintain ongoing behavioral and somatic health compliance, ensuring the well-being of program participants.
  6. Secure community resources to support program participants and their families, fostering a network of support.
  7. Monitor supported individuals for signs of relapse, providing timely interventions to prevent setbacks.
  8. Provide vocational counseling and job coaching, empowering individuals to achieve their career aspirations.
  9. Complete thorough job/task analysis, developing training plans to enhance employability.
  10. Document interactions, progress, and outcomes in case records, maintaining accurate and comprehensive documentation.
Requirements

A minimum Associate's degree in human services is required, along with Certified Peer Recovery Specialist certification or the ability to obtain it within 90 days of employment. A strong background in social work, behavioral health, addiction counseling, crisis management, mental health counseling, and motivational interviewing is also essential.
