Physical Therapist Assistant

We are seeking a skilled Physical Therapist Assistant to join our team in a Home Health setting. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our team.
Key Responsibilities- Assist physical therapists in implementing individualized treatment plans for patients in medical settings.
- Provide direct physical therapy services to patients, focusing on improving mobility, strength, and function.
- Collaborate with healthcare teams to support patients' rehabilitation goals and ensure coordinated care.
- Conduct therapeutic exercises and activities tailored to patients' needs and treatment goals.
- Monitor patients' progress and communicate findings to physical therapists, adjusting treatment interventions as necessary.
- Assist in the setup and use of therapeutic equipment and modalities during treatment sessions.
- Educate patients and caregivers on home exercise programs and strategies for maximizing functional independence.
- Document patient progress and treatment interventions accurately and in compliance with regulatory standards.
- Participate in discharge planning and transition of care processes as part of the interdisciplinary team.
- Maintain professional competence through continuing education and training activities.
- Associate's degree from an accredited Physical Therapist Assistant program.
- State licensure or certification as a Physical Therapist Assistant.
- Experience working in a medical setting such as skilled nursing facilities (SNFs), home health, outpatient clinics, or acute rehabilitation centers preferred.
- Knowledge of physical therapy interventions, techniques, and equipment used in medical settings.
