Job Summary:

We are seeking a skilled Nurse Care Coordinator to join our team at Medustrial Healthcare Staffing Service. As a Nurse Care Coordinator, you will be responsible for overseeing the health and well-being of our clients, providing guidance and support to their families, and collaborating with our healthcare team to ensure the highest level of care.

Key Responsibilities:

  • Health Assessment and Monitoring: Observe and assess clients' health records, symptoms, and reactions to treatment and care, and take necessary actions to ensure their safety and well-being.
  • Nursing Care Standards: Support nursing care standards by adhering to existing policies and procedures, and ensuring that all care is provided in a safe and therapeutic environment.
  • Communication and Coordination: Communicate effectively with clients, their families, and other members of the healthcare team to ensure that all care needs are met and that clients receive the highest level of care.
  • Vital Sign Monitoring: Measure and record clients' vital signs, such as height, weight, temperature, blood pressure, pulse, and respiration, and report any abnormalities to the physician and other healthcare team members.
  • Basic Care and Treatments: Provide basic care and treatments, such as taking temperatures or blood pressures, dressing wounds, treating bedsores, giving enemas or douches, rubbing with alcohol, massaging, or performing catheterizations, as needed.
  • Abnormal Diagnostic Data: Identify abnormal diagnostic data and report it to the physician and other healthcare team members, and incorporate the information into the nursing care plan.
  • Healthcare Team Collaboration: Work collaboratively with the healthcare team to assess clients' needs, plan and modify care, and implement interventions to ensure the highest level of care.
  • Evaluation and Outcomes: Evaluate the outcomes of nursing interventions and confer with other healthcare team members to ensure that all care is effective and efficient.
  • Communication Liaison: Serve as a communication liaison between the subsidiary and external healthcare providers, as needed.

Requirements:

  • Education and Licensure: Possess a valid, state-specific Registered Nursing license (BA in Nursing preferred).
  • Certifications: Must possess a current BLS certification.
  • Experience: One to three years' experience as a RN or three to five years' experience as a LPN.
