Responsibilities:
  • Conduct thorough health assessments and diagnose a wide range of medical conditions, including chronic diseases such as diabetes, hypertension, and heart disease.
  • Order and interpret diagnostic tests, prescribe medications and treatments, and develop individualized treatment plans.
  • Educate patients about their health conditions and medications, and coordinate care with other healthcare providers.
  • Participate in patient care quality improvement initiatives and maintain accurate and complete patient records.
Qualifications:
  • Master's degree in nursing with a focus on advanced practice nursing (APRN).
  • Certified as an APRN by the American Nurses Credentialing Center (ANCC) in internal medicine.
  • Current license to practice as a registered nurse in the state of NH.
  • Experience in providing primary care to adults of all ages.
  • Excellent communication and interpersonal skills.
