Responsibilities:- Conduct thorough health assessments and diagnose a wide range of medical conditions, including chronic diseases such as diabetes, hypertension, and heart disease.
- Order and interpret diagnostic tests, prescribe medications and treatments, and develop individualized treatment plans.
- Educate patients about their health conditions and medications, and coordinate care with other healthcare providers.
- Participate in patient care quality improvement initiatives and maintain accurate and complete patient records.
- Master's degree in nursing with a focus on advanced practice nursing (APRN).
- Certified as an APRN by the American Nurses Credentialing Center (ANCC) in internal medicine.
- Current license to practice as a registered nurse in the state of NH.
- Experience in providing primary care to adults of all ages.
- Excellent communication and interpersonal skills.
