Nova Health is seeking a highly skilled Clinical Pharmacist to join our ambulatory pharmacy team. As a key member of our team, you will be responsible for delivering optimal medication management across all dimensions of care.
Responsibilities- Partner with NHMG providers and clinical staff to provide exceptional patient care.
- Develop and implement medication therapy management plans and provide patient counseling.
- Promote high-quality, evidence-based, cost-effective pharmaceutical care.
- Precept residents and students, and participate in pharmacy and NHMG committees.
- Contribute to process improvement projects and support the educational and professional development of the department and system.
- 4-year Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.
- Postgraduate year 1 residency or fellowship, or two years of experience as a registered pharmacist in health system pharmacy, required.
- Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date.
- Board of Pharmacy Specialties Certification or other approved certification, required.
